California Healthy Kids Survey
7th Grade California Healthy Kids Survey - English The California Healthy Kids Survey (CHKS) is a comprehensive youth self-reported data collection system that provides anonymous but essential health risk assessment and resilience information to schools, districts, and communities. RUSD administers the survey to 5th and 7th grade students annually. Parent permission forms are sent home prior to the survey. You may view the survey by clicking on the links below:

Targeted at grades 5 and 7, the CHKS enables schools and communities to collect and analyze valuable data regarding local youth health risks, assets, and behaviors. The survey is a powerful tool that can help schools and districts meet all the assessment requirements of the Elementary and Secondary Education Act (ESEA) by accurately identifying areas of strength and weakness. It is designed to be part of a comprehensive data-driven decision making process to help guide the development of more effective health, prevention, and youth development programs.
At the heart of the CHKS is a research-based “Core” module that provides valid indicators of drug use, violence, crime, and physical and mental health. The Resilience & Youth Development Module which collects all the data needed for program compliance and allows comparability to state and national data has been integrated into the core module.
